Dr. Berry is a board-certified endocrinologist who specializes in personalized medical treatment of hormonal condition, integrating nutrition and lifestyle coaching.
Evergreen Endocrinology & Wellness is a direct specialty care practice, which means we do not accept or bill health insurance for services. In a direct specialty care practice, patients pay a fee directly to the healthcare provider in exchange for enhanced access to services and expertise. Removing insurance company restrictions allows a direct and personalized relationship between you and your doctor.
If you have a health savings account (HSA), you may be able to use those funds to pay your fees (check with your HSA). You may still use your health insurance to pay for services provided by outside vendors for labs, imaging, medications, and durable medical equipment.
Our Services
Initial consultation
Price: $350
All patients will need an initial consultation to establish care with Dr. Berry.
This visit will include:
Gathering and reviewing necessary outside records.
A scheduled visit with Dr. Berry of 60 minutes.
A detailed review of your medical history, outside records, imaging, nutrition status, currents concerns and symptoms, and a physical exam.
If you use a continuous glucose monitor or insulin pump, it will be downloaded for the visit.
At the end of this visit:
Laboratory and imaging tests will be ordered, as needed.
If a new medication is decided upon, it will be prescribed.
A follow-up visit (by phone/telehealth/in person) will be scheduled to review testing and determine next steps. At that time, you can decide whether to become a member or follow up as needed. This single follow-up visit is included in the consultation fee.

evergreen membership follow-up
Price: $105 per month
Membership highly recommended in order to achieve the best long-term results. One monthly price includes unlimited visits and covers all of the clinic’s service between visits.
What is included in membership:
Unlimited Appointments: You can choose in-person, virtual, or phone visits, as often as you need. If you have an urgent issue, we offer appointments in the same week - often on the same day.
Holistic care: Dr. Berry will manage all the endocrine conditions and take the lead in advising you on lifestyle factors impacting or potentially impacting your health. While I do not serve as a primary care doctor, I will act as a full-time medical advisor and be the most available physician you are likely to ever meet. Because of your increased access to Dr. Berry, there will be more opportunity to focus on your long-term wellness, without judgement or guilt.
Care coordination: Dr. Berry will assist with care coordination with primary care, other specialists, hospitals, ERs, etc. When necessary, Dr. Berry will provide high-quality well-vetted referrals to the best medical services he can find for you.
Advanced medical education: Dr. Berry loves to teach, and his goal is to help patients understand their bodies, how to navigate the healthcare system, and how to understand your own unique “treatment philosophy”.
Direct Communication: Open communication and a direct relationship with your doctor is key to maintaining your well-being. As a member, you can easily contact Dr. Berry by text or email during office hours.
Patient Portal Access: Members can use our patient portal without any extra charge. This secure platform lets you ask for prescription refills, and talk securely with Dr. Berry or a member of his team.
Complimentary CGM Downloads, Placement, and Training: For people with diabetes, continuous glucose monitoring (CGM) gives both of us the best insight possible into understanding and mastering your physiology. Once you set up a remote link with your device, Dr. Berry can remotely review and interpret data from continuous glucose monitors ($50 value) as well as placement and training for CGMs ($100 value) at no charge, even if needed between visits.
Remote pump review: People with insulin pumps know how important it is for their endocrinologist to review pump settings and performance. Once you set up a remote link with your device, Dr. Berry will be able to review your pump remotely as needed, allowing you to get help without having to come to clinic to have the device downloaded.
Expedited Prior Authorization and Forms: Dealing with insurance companies to secure access to testing and prescription medications is becoming increasingly complex. Dr. Berry will handle your insurance prior authorizations (PAs), appeals, and other necessary forms without charging you extra. Dr. Berry is highly experienced and savvy in advocating for your best interests with your insurance, and can often find a solution for coverage where other doctors fail.
No Commitment Required: We understand that your health needs can change, and we respect your choices. While we don't require a long-term commitment, we encourage continuous membership for consistent, high-quality endocrine care tailored to you. Should you ever decide to rejoin after a break, please note that there will be a re-enrollment fee of $350.
What is not included in membership:
Evergreen membership is not health insurance. The cost of others healthcare products and services (laboratory testing, imaging, prescription drugs) would still be your responsibility and are typically covered by your health insurance. For people without health insurance, Dr. Berry will help you find labs/imaging/medications at the lowest cost possible.
non-membership follow-up
(Fee for service)
Price: $300 per visit
While the membership model is recommended, this option is available for people with medical conditions needing minimal follow-up 1-2 times/year and minimal care between visits. Service in-between visits requires an additional charge.

30 minute clinic visit, in person or telemedicine
Review of labs and imaging obtained for the visit
CGM/pump download, if applicable
Medication prescription and refills
If additional testing is ordered at the appointment, Dr. Berry will review this over the phone.
This charge does NOT include service in-between appointment:
Prior authorization requests ($50 fee) or appeals ($50 fee)
Phone or portal communication with Dr. Berry ($300 per hour, billed in 10 minute increments)
CGM/Pump download between visits ($50), CGM placement with training ($100)
Completion of forms or medical letters ($25 each)
